Support is critical to those who have disabilities in keeping their homes or who have housing needs.

HSS:

Housing Stabilization Services

A Medical Assistance benefit to help people with disabilities and seniors find and keep housing. The program in Minnesota provides assistance to individuals and families experiencing homelessness or at risk of becoming homeless.The HSS program in Minnesota provides three services to support individuals and families experiencing homelessness or at risk of becoming homeless: Housing Consultation, Transition, and Sustaining.

ELIGIBLE MEMBERS

  •  Be on Medical Assistance (MA)

  •  Be 18 years old or older.

  • Adults who have a waiver such as CADI, DD, BI, EW or CAC waiver

  • Have a documented disability or disabling condition.

  • Be experiencing housing instability or homelessness

HOUSING CONSULTATION

helps individuals develop a personalized plan for housing stability and connect them to non-housing resources.

HOUSING TRANSITION

assists in finding suitable housing.

HOUSING SUSTAINABILITY

provides ongoing support to maintain housing stability.
